To get a 40+ most of your subjects would need to be 6s or 7s (as someone pointed out, for most subjects the average is between 4 and 5). The way the grade boundaries are decided make getting higher grades harder.

There’s an official examiners report on the IBO website – for November 09 the top 9.6\% got 40 and above. The number was 3.7\% for May 09, so apparently Austrialians/East asians are doing a great job. I don’t think the bulletin has stats by country.
