What does HR will get back to you mean?

This quote from the interviewer is a sign of hope which means probably you have made it to the round of interview and you will be informed about the process that is to be continued later.

What does it mean when a recruiter wants to discuss next steps?

It means that you are through to the next stage and they probably want to take up references or get you to do some assessments (ie the next steps). However, it means that they have some feedback about how you did in your interview which you may need to consider or that may help you with the next stage.
